Procedure for Nomination

You may either submit all parts of this process electronically, submitting the one- and five-page documents as e-mail to Dr. Bushman, using the link at the bottom of the page and using the interactive form below, or you may submit them all by traditional mail, printing the form below and typing in the required information.

1. Submit a one-page letter of nomination.

2. Fill out the information below and attach to the letter or hit the submit button at the bottom of the form. 

3. Prepare a five-page support document to be sent with the letter of nomination. ONLY FIVE PAGES -- ADDITIONAL PAGES WILL BE DISREGARDED.

4. Send nomination material to:
