Form for Attachment to Writing Submissions

To submit student's work do the following:

	complete the form on the screen
	print the form out
	check to see that writing is relevant to current topic
	attach the form to the student's writing

The student's name should not be on the work that is submitted. All parts of this form must be filled in; if any part is not completed, the writing will not be accepted. All parts of the form must be legible, if not, it will be discard.
